Job summary

YAI - Seeing beyond disability in New York City seeks a dedicated Registered Nurse to provide compassionate, person-centered care to individuals with I/DD across assigned programs and systems.

Under supervision, you will assess needs, develop nursing care plans, monitor treatments, educate patients and staff, and coordinate with external providers. This is a full-time, on-site role requiring an active NY/NJ RN license and BLS, with on-call rotation.

Qualifications

• Valid, active RN license in NY or NJ; registration to practice as an RN.
• Current Basic Life Support (BLS) certification for Healthcare Providers.
• Excellent interpersonal, verbal and written communication skills.
• Ability to lead, direct and teach program staff on medical support and specialized care.
• Proficiency with computers and electronic systems (EHR, eMAR, Workday, etc.).
• Complies with HIPAA and related privacy regulations.

Responsibilities

• Provide nursing services for persons supported in assigned programs, collaborating with direct care staff and leadership.
• Develop and update plans of nursing care and related medical support plans.
• Assess health status, monitor treatments, and evaluate progress with person-centered goals.
• Educate patients and staff on treatments, medications, diets, and healthy lifestyles.
• Manage intake assessments and coordinate medical appointments and referrals.
• Oversee nursing documentation, on-call coverage, and liaison with external providers.
