Responsibilities

• Collaborates with the clinical team to assess patients likely to benefit from care coordination.
• Facilitates discharge planning and ensures continuity of care across inpatient, ER and ambulatory settings.
• Engages patients and families, builds care plans, and supports self-management education.
• Uses IT tools to support disease registry, outcome tracking and preventive reminders.
• Advocates for patient safety and helps navigate healthcare delivery and reimbursement systems.
