Qualifications

• Bachelor's Degree in Nursing or Social Work required.
• RN license preferred; LVN/LPN license acceptable.
• Two years clinical nursing experience preferred.
• Knowledge of Medicare, Medicaid programs and benefits.
• CCM certification a plus; continuing education/licensing required.

Responsibilities

• Communicate regularly with residents and families about care plans and treatment protocols.
• Attend morning meetings to coordinate with the care team.
• Answer residents' questions about care, illness progression, and plans.
• Monitor and adjust resident statuses based on changing needs.
• Organize daily work and prioritize needs of residents and discharges.
• Complete required documentation and perform utilization review.
• Coordinate care with residents, providers, facilities, and payers.
• Oversee admissions, discharges, and referrals to/from the facility.
• Maintain professional, respectful interactions with residents, families, and staff.
