Qualifications

• Registered nurse with NH license (or compact license) required.
• Graduate of an accredited school of nursing.
• CPR/BLS certification required.
• Strong computer keyboarding and data entry skills.
• Minimum 3 years of professional nursing experience.
• Valid driver’s license to travel to off-site locations.
• Experience with substance use disorders and case management preferred.

Responsibilities

• Perform triage and rapid clinical assessments at shelter-based clinics with the HCH team.
• Initiate emergency response to stabilize patients and notify providers.
• Assess and document vital signs and chief complaints for clinic visits.
• Conduct comprehensive bio-psycho-social assessments and SUD screenings.
• Develop individualized care plans and provide health education and counseling.
• Coordinate referrals and communicate assessment findings to providers.
• Ensure EMR documentation is current for clinic visits.
• Coordinate discharge plans and transportation/translation support as needed.
• Participate in Street Medicine with HCH staff to engage housing insecure individuals.
