Job summary

Boston Health Care for the Homeless Program (BHCHP) is seeking a Registered Nurse for per diem weekday coverage in the greater Boston area. The role focuses on care coordination for the OneCare program, including UCA assessments and personalized care plans for clients eligible for Medicare and Medicaid.

Ideal candidates are licensed in Massachusetts with 1–2 years of RN experience, strong assessment and communication skills, and familiarity with EPIC, Guiding Care, and ECW EMRs.

Qualifications

• Strong medical and surgical assessment skills.
• Interest in working with disadvantaged groups; understanding of substance abuse and mental health.
• Excellent organizational and interpersonal skills; clear communication via phone, email, and secured messaging.
• Able to work as part of a multidisciplinary team and autonomously.
• Able to prioritize tasks and manage timetables.
• Experience documenting in EMRs: EPIC, Guiding Care, and ECW.
• Proficient with Microsoft Office Suite and adaptable to technology.
• Well organized, efficient, and able to work both independently and in a team.
• Driver’s license a plus.

Responsibilities

• Coordinates care between BHCHP medical teams and the OneCare program.
• Performs UCA assessments and helps create care plans.
• Outreaches to clients for face-to-face contact in the greater Boston area.
• Uses EMR to record interactions and update authorizations.
• Maintains BHCHP standards of patient rights, safety, and culturally competent care.
• Flexibly assists with other projects as needed.
