Qualifications

• Graduate from an accredited Nursing Program.
• Bachelor's degree with 3 years of clinical experience.
• Recent acute care hospital experience preferred.
• Previous Case Management experience preferred.
• Strong communication, collaboration, adaptability.

Responsibilities

• Facilitates the development of a care plan in collaboration with the patient, family, caregiver and health care team.
• Advances the care plan to achieve outcomes and monitors progress.
• Triages telephone calls and answers questions within a registered nurse’s scope.
• Communicates information to the health care team and patient’s care.
• Advocates for services and insurance benefits within regulations.
• Recognizes situations requiring referrals and discusses with leadership.
• Evaluates options balancing cost and quality to optimize outcomes.
• Evaluates patient outcomes and participates in quality improvement initiatives.
• Coordinates care with clinical research for patients on complex protocols.
• Performs other duties as required.
