Boston Health Care for the Homeless Program is seeking a Registered Nurse to support the OneCare care coordination program. The role focuses on UCA assessments, care plan development, and coordinated services for clients in the greater Boston area.

The position requires MA RN licensure, experience in primary care or community health, and proficiency with EMR systems. The role offers flexible, largely in-person work with some remote hours in a hybrid setup.

Competenze

• Licensed RN in Massachusetts is required.
• Experience in primary care or community health settings preferred.
• Proficient with EMR documentation (EPIC, Guiding Care, ECW).

Mansioni

• Conduct UCA assessments for OneCare program participants.
• Work as part of the primary care team with MDs, NPs/PAs, and other clinicians.
• Perform outreach to clients in the greater Boston area to meet face-to-face; update care plans.
• Document interactions and outcomes in EMR accurately and timely.
• Assist with transitions of care and coordinate services like visiting nursing and social supports.
