Job summary

Care For the Homeless (CFH) in New York City is hiring a Registered Nurse to join the PCMH team in a multidisciplinary setting. The RN will provide clinical support across medical and behavioral health care, administer vaccines, manage patient flow, and document care per NYS standards.

Qualifications

• Current NYS Registered Nurse licensure and BLS Certification are required.
• Ambulatory care, ER, or physician office experience is preferred.
• BSN degree is preferred but not required.

Responsibilities

• Provide patient intake and screening; administer vaccines and medications.
• Perform examinations and assist practitioners as needed.
• Maintain nursing documentation and patient records in EMR.
• Ensure infection control and safety according to DOH OSHA standards.
• Support care coordination and education for patients and families.
