Job summary

Qualifications

• Minimum of an associate degree in nursing or diploma in nursing.
• BSN in progress or plan to obtain within 3 years for hires after 2019.
• Six months of relevant nursing experience or participation in Nurse Residency.

Responsibilities

• Assess patient health data and plan individualized care.
• Coordinate plan of care with patient, family, and care team.
• Provide direct nursing care following evidence-based practices.
• Document care and communicate patient status to the team.
• Support safe patient transitions and discharge planning.
