Job summary

Northwell Health is seeking a registered nurse to deliver age-appropriate, evidence-based care across a designated patient group. You will collaborate with physicians and the health team to coordinate procedures, delegate activities, and ensure safe, effective nursing care through the full nursing process.

Responsibilities include conducting comprehensive assessments, developing outcome-driven care plans, implementing treatments, and communicating with patients and families to support discharge

Qualifications

• Graduate from an accredited School of Nursing; Bachelor's Degree preferred.
• Current license to practice as a Registered Professional Nurse in New York State.
• CPR/BLS certification; ACLS preferred.

Responsibilities

• Assess physiologic health status and review patient charts; document plan of care.
• Assess psychosocial health status; identify needs and coordinate support services.
• Identify patient outcomes and establish individualized goals and teaching needs.
• Formulate and prioritize the care plan; coordinate medications, supplies, and procedures.
• Implement and monitor care, ensure safety, and reassess as patient needs change.
• Collaborate with physicians and health team members and provide guidance to care staff.
• Perform Decubitis care and wound/skin care per facility procedures.
• Administer medications safely and provide patient/family education.
