Job summary

Wellstar Health System in Georgia is seeking an RN Care Coordinator to assess transitional care needs, coordinate services across the care continuum, and guide patients and families through the discharge process. You will partner with physicians, utilization review nurses, and interdisciplinary teams to remove barriers to discharge and ensure safe transitions.

Active RN license, at least one year in acute care, and current BLS are required; a sign-on bonus up to $10,000 and relocation assistance
