Job summary

Ohio State University Physicians is seeking a Clinical Nurse Care Coordinator in Columbus to deliver evidence-based, patient-centered care and keep patients up to date on their care plans and goals. You will work with clinic teams and focus on high-risk medically complex patients, disease registries, and care transitions.

As an RN with an Ohio license and CPR/BLS certification, you will liaise with families and other agencies to coordinate comprehensive care, while staying current with standards

Qualifications

• Associate degree in nursing from an accredited program.
• Current Ohio RN license.
• CPR/BLS certification.
• Knowledge of triage procedures in clinic, physician office or urgent care.
• Ability to educate and clearly communicate with patients and families about disease and care.

Responsibilities

• Collaborate with clinic team to provide evidence-based patient-centered care and keep patients up to date on care plans and related goals.
• Focus on high-risk medically complex patients, disease registries, chronic disabilities and care transitions.
• Act as liaison with patient’s family and other agencies to facilitate improved communication and provide comprehensive whole-person care.
• Stay up to date on standards and assist with programs and processes.
