Ohio State University Physicians is seeking a Clinical Nurse Care Coordinator to join a compassionate, patient-centered care team. You will collaborate with clinic staff to manage care plans, focusing on high-risk medically complex patients and those transitioning between care settings.

You will liaise with patients and families, maintain current nursing standards, and help implement programs that promote quality, safety, and outcomes.

Qualifications

• Associate Degree in Nursing from a CCNE or ACEN-accredited program.
• Current Ohio Registered Nurse license.
• CPR/BLS certification.
• Knowledge of triage procedures with clinic, physician office and/or urgent care experience.
• Ability to maintain quality, safety, and infection control standards.
• Knowledge of medication administration procedures and patient care aids.
• Ability to educate and clearly communicate with patients and families about disease and care.
• Excellent written and verbal communication skills.
• Strong interpersonal skills to handle sensitive and confidential situations.
• Demonstrated poise, tact and diplomacy; analytical thinking to interpret data.
