Job summary

Ohio State University Physicians is seeking a Clinical Nurse Care Coordinator to provide patient-centered care for high-risk patients across clinics. You will help patients stay on their care plans and coordinate with families, care facilities, and others to improve outcomes.

This RN role emphasizes evidence-based practice, standard updates, and collaboration with multidisciplinary teams to support transitions from pediatric to adult care and post-discharge follow-up.

Qualifications

• RN with an Associate Degree required.
• CPR/BLS certification required.
• Knowledge of triage procedures and clinical care settings.
• Ability to educate patients and families clearly.
• Strong written and verbal communication; maintain confidentiality.

Responsibilities

• Collaborate with clinic teams to provide evidence-based, patient-centered care.
• Focus on high-risk medically complex patients and care transitions.
• Act as liaison with families and other agencies to improve care coordination.
• Stay up to date on standards and assist with program/process improvements.
