Job summary

The Clinical Nurse Care Coordinator at Ohio State University Physicians collaborates with clinic teams to provide evidence-based, patient-centered care and help patients stay up to date with their care plans.

This role focuses on high-risk medically complex patients, those in disease registries, chronic conditions, and care transitions, serving as a liaison with families and other agencies to ensure safe, coordinated, whole-person care and ongoing adherence to standards.

Qualifications

• Associate Degree in Nursing from a CCNE or ACEN accredited program.
• Current Ohio Registered Nurse license.
• CPR/BLS certified.
• Knowledge of triage procedures and clinic, physician office and/or urgent care experience.
• Ability to maintain quality, safety, and infection-control standards.
• Knowledge of medication administration and patient care aids.
• Ability to educate and clearly communicate with patients and families about disease and care.
• Ability to convey information in writing and verbally.
• Strong interpersonal skills to handle sensitive situations; poise and diplomacy.
• Analytical thinking to gather and interpret data and implement solutions.
