Job summary

Qualifications

• Graduate of an accredited school of Nursing.
• Two years of hospital or community-based patient care nursing experience.
• Current RN license in Texas or valid compact license.
• BLS for Healthcare Providers certification required.

Responsibilities

• Assess patients on the Care Coordination team to develop plans of care.
• Identify funding sources and eligibility for patient care.
• Collaborate with physicians and staff to implement the plan of care.
• Monitor discharge planning and ensure timely patient transitions.
• Support utilization management and communicate with payers.
