Job summary

The University of Texas Southwestern Medical Center in Dallas is seeking an experienced RN Case Manager to coordinate care for defined patient populations within an advanced care setting.

You will lead multidisciplinary teams, implement clinical pathways, manage transitions, and ensure timely discharge planning with patient and family education.

Required: BSN, Texas RN license, and at least 2 years of acute care hospital experience.

Qualifications

• Bachelor's degree in nursing from an accredited program.
• Current Texas RN license.
• 2+ years of RN experience in an acute care hospital setting.

Responsibilities

• Lead multidisciplinary team developing, refining, updating, and communicating plan of care for defined patient population.
• Implement clinical pathways including educational needs of patients/families and monitor deviations.
• Coordinate care with care team to maximize patient throughput and shorten length of stay.
• Partner with social work and utilization review to maximize resources for patient and family.
• Ensure discharge planning and follow-up appointments are documented and understood by patient/family.
