Job summary

Texas Children’s Hospital is seeking a Care Coordinator to provide collaborative, comprehensive case management across the care continuum. You will work with an interdisciplinary team and the patient/family to optimize outcomes, ensure appropriate resource use, and support the physician in care planning.

The role requires nursing eligibility and active RN licensure in Texas, with strong communication skills and experience in acute care or pediatric settings.

Qualifications

• Bachelor’s degree in nursing or ADN with BSN progression.
• Current RN licensure by the Texas Board of Nursing or a multi-state compact.
• BLS certification from the American Heart Association.
• Minimum 3 years of nursing experience in an acute care setting.
• Experience in community health, pediatric care, home care, case management, managed care, or utilization review preferred.

Responsibilities

• Assess, develop, implement and monitor a comprehensive plan of care with an interdisciplinary team and family.
• Review clinical documentation with medical and nursing staff to ensure appropriate level of care and patient placement.
• Identify problems and discuss with the team to address barriers to care.
• Serve as content expert for level of care and coordinate resource utilization.
• Screen patients for level of care and ensure accurate chart documentation.
• Coordinate timely transfer of information to unit Care Coordinator for revenue cycle support.
• Complete visits with patients across the care continuum as indicated.
• Participate in orientation and training of new department members.
• Identify and monitor quality improvement opportunities across care continuum.
• Identify and report quality, safety and service issues to Quality Management.
• Demonstrate professional communication with patients, families, peers, and health care team members.

Education

Bachelor’s degree in Nursing

RN Licensure (Texas Board of Nursing or compact)

BLS (American Heart Association)

3 years acute care nursing experience

Experience in community health / pediatric care / case management / utilization review
