MLK Community Healthcare is seeking an RN Coordinator for the Enhanced Care Management (ECM) program. The role focuses on coordinating care for ECM enrollees, leading the interdisciplinary team, and ensuring timely assessments, care plans, and follow-ups with primary and behavioral health providers.

The RN Coordinator will supervise a patient panel, advance medication management, and support transitions across care settings, including housing and community services as needed.

Qualifications

• Experience in case/care management preferred.
• Ability to build strong relationships with peers and leaders.
• Demonstrated ability to coordinate care across multiple providers.

Responsabilités

• Provides Clinical Oversight of ECM Lead Care Managers to address patient medical and behavioral health needs.
• Participates in developing patient-centered care plans for enrollees on their panel.
• Manages a patient panel of 15-20 patients.
• Ensures ECM members receive needed services and track progress toward goals.
• Oversees quality measures across access, clinical, and experiential benchmarks.
• Collaborates with Care Managers to review medical visit summaries and scheduling.
