Job summary

MLK Community Healthcare is seeking an RN Coordinator to support the Enhanced Care Management (ECM) program. The role requires coordinating care for enrolled patients, leading care teams, and aligning services with patient-centered goals across medical and behavioral health disciplines.

The RN Coordinator will participate in Interdisciplinary Care Team meetings, perform nursing assessments, and help develop care plans, ensuring timely follow-up and adherence to treatment plans.

Qualifications

• BSN preferred; Current RN license in California.
• 1 year of case/care management experience preferred; strong relationship-building with peers and leaders.

Responsibilities

• Provide clinical oversight of ECM Lead Care Managers.
• Develop patient-centered care plans for enrollees.
• Coordinate care across primary, behavioral health, and specialists.
• Conduct nursing assessments and coordinate transitions in care.
• Engage with vulnerable populations and community partners to address social determinants of health.
