MLK Community Healthcare seeks an RN Coordinator for the Enhanced Care Management program. The role coordinates care for ECM enrollees, leads the interdisciplinary team, and develops individualized care plans to meet health goals.

Qualifications

BSN preferred.
Current California RN license.
Minimum 1 year of case/care management experience.
Experience with homelessness or community health is a plus.
Strong organizational and problem-solving skills.

Responsabilités

Provides clinical oversight of ECM Lead Care Managers.
Develops patient-centered care plans for enrollees on their panel.
Manages a patient panel of 15-20 individuals.
Coordinates care with PCP, behavioral health clinician, and specialists.
Engages with community organizations and addresses Social Determinants of Health.
Assists with transitions in care and aims to prevent readmission.
Performs medication reconciliation and supports adherence.
Assists with billing submission.
Conducts outreach and home visits as needed.
