Martin Luther King, Jr. Community Hospital seeks an RN Coordinator to support the Enhanced Care Management program. The role coordinates care plans for patients, participates in interdisciplinary team meetings, and ensures timely follow-up with PCPs, behavioral health providers, and specialists.

The RN Coordinator conducts nursing and social assessments, develops care plans, and leads transitions to appropriate care settings, with oversight of patient panels and care quality.

Competenze

• BSN preferred and active California RN license required.
• Minimum 1 year of case/care management experience preferred.
• Demonstrated ability to build relationships with peers and leaders.
• Ability to coordinate care with medical assistants and other staff.

Mansioni

• Provide clinical oversight of ECM lead care managers and patient care plans.
• Participate in interdisciplinary care team meetings for medication management and follow-up.
• Conduct nursing assessments, address behavioral health and social service needs.
• Coordinate transitions between hospital-based care and community-based services.
• Support quality measures and ensure patients receive needed services.
• Assist with billing submissions and chart audits.
