Job summary

MLK Community Healthcare in Los Angeles, CA is seeking an RN Coordinator for the Enhanced Care Management program. Qualifications

• Minimum of one (1) year of case/care management experience preferred.
• Demonstrated experience building effective, strong relationships with peers and leaders.
• Current Basic Life Support (BLS) certification from the American Heart Association.
• Current Advanced Cardiac Life Support (ACLS) for Health Care Providers, preferred.

Responsibilities

• Provides Clinical Oversight of ECM Lead Care Managers to address medical and behavioral health needs.
• Participates in developing patient-centered care plans for enrolled patients (panel of 15–20).
• Ensures ECM members receive needed services and measures progress toward goals.
• Oversees quality measures across access, clinical, and experiential benchmarks.
• Assists with billing submission and health plan chart audits.
• Performs other duties as assigned or requested.
