Job summary

Martin Luther King, Jr. Community Hospital seeks an experienced RN Coordinator to lead the Enhanced Care Management (ECM) program.

You will coordinate the care of ECM enrollees, participate in Interdisciplinary Care Team meetings, and develop patient-centered care plans with attention to medication management and follow-up with primary and behavioral health providers. The role requires strong leadership, nursing assessments, and a commitment to serving underserved populations.

Qualifications

• BSN preferred and California RN license required.
• Minimum 1 year of case/care management experience preferred.
• Demonstrated ability to build effective relationships with peers and leaders.

Responsibilities

• Provides clinical oversight of ECM lead Care Managers to address medical and behavioral health needs.
• Develops patient-centered care plans for enrollees on their panel.
• Manages a patient panel of 15-20 patients.
• Ensures ECM members receive needed services and monitors progress toward care plans.
• Oversees quality measures across access, clinical, and experiential benchmarks.
• Coordinates care transitions and collaborates with PCPs, BH providers, and specialists.
• Provides health education for acute conditions, chronic diseases, and medication management.
• Engages vulnerable populations through multidisciplinary outreach, including home visits and community outreach.
• Assists with scheduling, billing submission, and chart audits as needed.
