Job summary

MLK Community Healthcare in Los Angeles seeks a Registered Nurse (RN) Coordinator for the Enhanced Care Management program. You will coordinate care for ECM enrollees, participate in Interdisciplinary Care Team meetings, manage assessments, care plans, and align with PCP, BH providers, and specialists to meet health goals.

You will lead workflows, prioritize based on acuity, collaborate with partner organizations, and support transitions to primary care or community services, including housing

Qualifications

• BSN preferred and a current California RN license.
• Minimum of one year case/care management experience.
• Experience building relationships with peers and leaders.

Responsibilities

• Provide clinical oversight of ECM lead care managers.
• Develop patient-centered care plans for enrollees (panel of 15–20).
• Ensure ECM members receive needed services and measure progress.
• Oversee coordination of hospital and community care plans.
• Perform timely medication reconciliation and support adherence.
• Engage in multidisciplinary outreach including home visits and community referrals.
