Responsibilities

• Provide competent nursing care in a safe and consistent manner.
• Respond to incoming telephonic requests with reliable service.
• Provide triage and symptom management, in-person or remote as needed.
• Review medications and manage prescriptions and prior authorizations.
• Educate patients and families on chronic conditions and care plans.
• Collaborate with care team to monitor and adjust patient care plans.
• Foster a welcoming environment and coordinate care with caregivers.
• Participate in care team meetings and clinic operations.
• Deliver exceptional patient experiences with responsive and respectful care.
• Perform point-of-care testing and procedures as needed.
