Job summary

Oak Street Health is seeking a Registered Nurse to coordinate patient care and deliver competent nursing services in a busy clinic environment. You will collaborate with the care team, perform triage, education, and medications management while maintaining HIPAA standards and patient confidentiality.

Ideal candidates hold PA RN licensure, CPR/BLS certification, and EMR experience; strong communication and teamwork are essential.

Qualifications

• Active RN licensure in good standing with the state of Pennsylvania.
• Active CPR/BLS certification.
• Experience with Electronic Medical Record (EMR) systems.
• Clinic setting experience with hands-on patient care and injections/vaccines.
• Ability to maintain patient confidentiality and HIPAA compliance.
• Strong communication and assessment skills for patient care.
• Ability to work independently and manage multiple tasks in a team setting.
• US work authorization.

Responsibilities

• Provide competent nursing care with safety and consistency.
• Respond to telephonic requests and be reliable for patient needs.
• Deliver triage and symptom management for patients in/out of clinic.
• Follow standardized protocols for medication management and refills.
• Review and update patient medications; coordinate care plans.
• Educate patients and deliver direct care for chronic conditions.
• Collaborate with care team and monitor care plans across panels.
• Delegate tasks to colleagues to support patient needs.
• Build relationships with patients and caregivers to support preventative care.
• Create welcoming environment and ensure equitable care across populations.
• Participate in care team meetings and daily huddles.
• Deliver exceptional patient experience through service and responsiveness.
• Perform point-of-care testing and procedures including phlebotomy as needed.
