Job summary

Oak Street Health is seeking a Registered Nurse to build strong relationships with patients, coordinate care, and deliver seamless nursing services. You will apply standardized pathways to ensure timely, competent care with an emphasis on patient experience and team collaboration.

This role requires strong communication and clinical assessment across in-person and remote settings. You will report to the Practice Manager or Nurse Supervisor.

Qualifications

• Active RN licensure in good standing.
• BLS Certification.
• EMR experience.
• US work authorization.
• Ability to assess patients without face-to-face interaction.

Responsibilities

• Provide competent nursing care using standards and pathways.
• Respond to telephonic requests reliably and empathetically.
• Provide triage and symptom management, remote or in-person.
• Review medications and manage prescriptions and refills.
• Educate patients and deliver direct care for chronic conditions.
• Collaborate to monitor care plans across provider panels.
• Build relationships with patients and caregivers to support prevention.
