• BSN preferred; ADN or Diploma acceptable.
• AHA Basic Life Support preferred.
• CCM or other case management certification preferred within 3 years of hire.
• Minimum 3 years of acute clinical care experience preferred.

Responsabilidades

• Obtain pre-certifications and authorizations from third-party payers prior to hospitalization.
• Coordinate discharge planning and post-acute services with patients, caregivers, and care team.
• Document and report clinical denials/appeals; support action plans for denial trends.
• Monitor and verify data to evaluate interventions and outcomes.
• Communicate with outpatient providers to enhance continuum of care.
• Assist with utilization review changes and educate staff on guidelines.
