Qualifications

• RN license in state of practice (or compact) is required.
• Bachelor’s degree in nursing or social work preferred.

Responsibilities

• Coordinate clinical and psycho-social activities with the Interdisciplinary Care Transitions team and physicians.
• Monitor patient stays for effective care coordination and discharge planning.
• Refer high-risk patients for additional support as needed.
• Serve as patient advocate to support informed decisions by patient and family.
• Collaborate with staff to review goals, optimize resource use, and ensure continuity of care.
