Job summary

Wellstar Health System is seeking a Care Coordinator RN to assess transitional care needs, coordinate across the continuum, and engage with patients and families to ensure care needs are met. Qualifications

• Minimum 1 year nursing experience in the acute care setting.
• Excellent written and verbal communication skills; maturity, self-confidence and composure under pressure.
• Ability to work collaboratively with physicians, care team, and patient/family to progress care.

Responsibilities

• Assess patient chronic disease management needs and psychosocial risk factors.
• Coordinate discharge planning with care team and payers to ensure smooth transition.
• Conduct patient/family education and participate in interdisciplinary rounds.
• Identify barriers to discharge and escalate to the appropriate leader for resolution.
• Document care plans clearly and timely in the medical record.
