Job summary

Qualifications

• Associates or Diploma in Nursing; BSN preferred.
• RN license in the appropriate state or compact.
• Experience in acute care and discharge planning is desirable.

Responsibilities

• Assess transitional care needs and coordinate care across the continuum.
• Engage with patient and family to ensure discharge goals are met.
• Collaborate with physicians, UM, PAS and care team to plan progression.
• Document assessments and progress in the medical record.
