Job summary

Wellstar Health System in Georgia is seeking a Care Coordinator RN to assess transitional care needs and coordinate care across the continuum. You will engage patients and families to ensure needs are met and work with physicians and the care team to promote efficient discharge planning.

This role requires 1+ year of acute care nursing, strong communication, and the ability to navigate complex care pathways within a large health system.

Qualifications

• Associates Degree in Nursing or Diploma required; Bachelors preferred.
• Experience in acute care nursing preferred; 1+ year strongly desired.
• Excellent communication and collaboration with care teams and patients.

Responsibilities

• Assess transitional care needs and coordinate care across the continuum.
• Engage with patient and family to assure care needs are met.
• Participate in interdisciplinary rounds and discharge planning.
• Identify barriers to discharge and work to resolve them.
• Document in EMR and communicate with physicians and care team.
