Job summary

Vanderbilt University Medical Center is seeking a seasoned nurse to coordinate scheduling, admission, transfer or consult of emergent, urgent and elective patients into accepting facilities with appropriate levels of care and timely transport. The role requires critical thinking to optimize patient outcomes and seamless cooperation with referring hospitals and receiving facilities.

The position advances patient flow through the continuum of care, requiring leadership, teaching and hands-on use

Qualifications

• Must hold Tennessee RN license and be a graduate of an approved nursing program.
• Minimum 5 years of relevant nursing experience.

Responsibilities

• Coordinate scheduling, admission, transfer and consultation of patients into an accepting facility with appropriate level of care.
• Assess patient acuity and make recommendations to optimize patient outcomes.
• Educate nursing and allied staff on level of care and admission/transfer issues.
• Ensure accurate data entry in the electronic health record (EHR) and protect PHI.
