Qualifications

• Bachelor's degree in Nursing is required.
• Licensed Registered Nurse (RN) in good standing.
• Minimum 3 years of relevant work experience in nursing or care coordination.

Responsibilities

• Coordinate patient and family interventions across the continuum of care.
• Develop case management plan to attain patient/family outcomes.
• Analyze progress on the plan of care and remove barriers using process improvement methods.
• Incorporate ethical considerations in nursing practice across care settings.
• Provide leadership to the interdisciplinary team and support care coordination goals.
