Job summary

Vanderbilt Health seeks a Case Manager to coordinate services across the continuum of care, balancing high-quality patient care with cost-effective resource use. You will work flexible weekend shifts, primarily Saturdays and Sundays, with potential occasional 12-hour days.

You’ll assess and plan patient care, coordinate admissions and discharges, and collaborate with care teams to ensure appropriate care levels and reimbursement.

Qualifications

• RN licensure in the relevant state.
• 2 years of relevant work experience.
• Graduate of an approved nursing program.

Responsibilities

• Coordinate admission, treatment, and discharge plans and ensure proper utilization of hospital services
• Ensure care is provided at the most appropriate level for meeting patient care needs
• Serve as a liaison to ensure appropriate reimbursement for hospital stays and services
