Job summary

Vanderbilt University Medical Center seeks an experienced RN to coordinate patient care across the continuum. You will assess complex needs, develop case management plans, and work with the care team to remove barriers and optimize resources.

You will contribute to nursing pillar goals including quality, patient satisfaction, and financial stewardship, while guiding patients and families through the care process.

Qualifications

• Bachelor's degree in nursing required.
• Current RN licensure in the United States.
• Experience with case management and care coordination is preferred.

Responsibilities

• Assess and identify patients with complex clinical needs requiring a coordinated plan of care.
• Develop and evaluate case management plans to achieve patient- and family-specific outcomes.
• Lead and participate in interdisciplinary team care to ensure efficient resource use.
• Uphold ethical provisions and contribute to pillar goals for quality and safety.
• Provide education and guidance to patients, families, and staff as needed.
