Job summary

Vanderbilt Health is seeking an experienced RN Care Coordinator to support care transitions and optimize patient outcomes. You will partner with clinicians to coordinate evaluation, discharge planning, and post‑discharge follow‑up, ensuring timely information exchange and patient education.

You will engage patients and families using teach-back methods, promote coordinated care across episodes, and participate in evidence‑based practice to improve quality and safety across the care continuum.

Qualifications

• LIC-Registered Nurse licensure required.
• Bachelor's degree in Nursing from an accredited program.
• Minimum 5 years of relevant nursing experience in care coordination or related field.

Responsibilities

• Coordinate evaluation of defined patient population and collaborate with the clinical team.
• Serve as liaison between referring providers and Vanderbilt Health to ensure smooth access to care.
• Lead discharge and transition planning with multidisciplinary teams to optimize care episodes.
• Develop and disseminate patient education materials and customize medication guidance as needed.
