Qualifications

• BSN preferred and California RN license required/preferred.
• Current BLS for Health Care Providers.
• ACLS preferred.
• Valid CA Driver's License for driving company vehicle.
• Experience in case/care management preferred.
• Experience with homelessness and community outreach preferred.

Responsibilities

• Provide clinical oversight of ECM Lead Care Managers and address medical/behavioral health needs.
• Develop patient-centered care plans for enrollees.
• Manage a patient panel of 15–20 patients.
• Ensure ECM members receive needed services and track progress toward goals.
• Oversee quality measures across access, clinical, and experiential benchmarks.
• Coordinate care with MAs and other staff to form a strong team.
• Participate in outreach and address social determinants of health.
