CommuniCare Family of Companies in Charlottesville, VA is seeking an RN or LPN MDS Nurse RAC to join our team. The role focuses on accurate and timely completion of MDS assessments and coordination of the RAI process, ensuring compliant care and proper reimbursement.

Responsibilities include overseeing Medicare/Medicaid case mix and coordinating with the clinical team to maintain quality outcomes. Licensure in the state and prior long-term care MDS experience are preferred.

Qualifications

• Graduate of accredited nursing program.
• RN or LPN licensure in the employed state.
• Three years in long-term care preferred.

Responsibilities

• Complete MDS assessments accurately and timely; coordinate RAI process.
• Ensure compliance with regulations and optimize reimbursement.
