Job summary

Chase City Health & Rehab Center is seeking an experienced MDS Coordinator RN to join our clinical leadership team in Chase City, VA. The RN will oversee the Resident Assessment Instrument (RAI) process, care planning, regulatory compliance, and reimbursement activities, coordinating with interdisplinary staff and families to ensure timely, accurate documentation.

The ideal candidate holds a Virginia RN license, has 2+ years of MDS experience in long-term care, and may pursue RAC-CT

Qualifications

• Current Virginia Registered Nurse license required.
• Minimum of two years of MDS experience in skilled nursing or long-term care.
• RAC-CT certification preferred or willingness to obtain.
• Strong knowledge of MDS, RAI process, Medicare/Medicaid, PDPM and regulatory requirements.
• Excellent assessment, documentation, and analytical skills.
• Strong organizational and time management abilities.
• Excellent communication, leadership, and customer service skills.

Responsibilities

• Coordinate and oversee MDS assessments, CAAs, and care plans.
• Collaborate with nursing, therapy, social services, dietary, physicians, residents, and families.
• Ensure MDS assessments, care plans, and documentation are accurate and timely.
• Maintain assessment schedules and communicate deadlines to the team.
• Review MDS for accuracy, completeness, and regulatory compliance.
• Ensure supporting documentation substantiates coding and reimbursement requirements.
