Job summary

Fundamental in South Carolina is seeking a Registered Nurse Assessment Coordinator (RNAC, MDS) to coordinate patient/resident care from admission through discharge.

You will identify/monitor resident skilled services, oversee the RAI/MDS processes, complete the MDS and develop the Plan of Care, and participate in ITM meetings.

Requires an active RN license, 2+ years in nursing and long-term care, RAI experience preferred, ICD-10 proficiency, and reliable transportation.

Qualifications

• Must hold an active RN license in the state of practice.
• Minimum 2 years of direct nursing experience.
• Minimum 2 years of long-term care experience.
• Preferably at least 1 year of RAI experience (MDS focus).
• ICD-10 coding proficiency required.
• Reliable transportation is required.

Responsibilities

• Identifying/monitoring resident skilled services.
• Coordinating the Resident Assessment Instrument (RAI) process.
• Completing an accurate MDS and development of the interdisciplinary Plan of Care.
• Monitoring patient/resident status to identify changes in condition, barriers to discharge, and progress toward care goals.
• Participating in daily Medicare Prospective Payment System (PPS) Interdisciplinary Team Meeting (ITM).
• Establishing and maintaining positive relationships with all members of the healthcare team and functioning as part of the nursing management team.
