Job summary

LaFayette Manor Inc seeks an experienced Registered Nurse Assessment Coordinator (RNAC)/Medical Records Coordinator in Uniontown, PA to support regulatory compliance and resident-centered care. This full-time position includes weekend on-call duties and potential to become permanent.

The role emphasizes survey readiness, accurate care plans, and strong communication, with responsibility for OBRA and Medicare/managed care scheduling and coordination with CHC case managers.

Qualifications

• Current LPN/RN license is required.
• Basic computer and EHR/PCC knowledge and experience is required.
• Preferred RAC-CT certification.
• Experience with Resident Minimum Data Sets and Care planning preferred.
• Long term care experience is highly preferred.
• Strong organizational and communication skills are required.

Responsibilities

• Coordinate development and accurate completion of the resident assessment and care plan in compliance with federal, state and local guidelines (MDS, RAI, care planning).
• Schedule OBRA and Medicare/managed care assessments and care plan reviews per regulatory guidelines.
• Inform residents and families of coverage status and coordinate with CHC case managers for Managed Medicaid providers.
• Participate in committees as directed.
