Armstrong Rehabilitation & Nursing Center in Kittanning, PA is seeking an RNAC Registered Nurse Assessment Coordinator to join our dedicated team. You will perform comprehensive resident assessments, identify medical, social, and psychological needs, and develop individualized care plans in collaboration with physicians, therapists, and social workers.

This full-time on-site role emphasizes regulatory compliance and accurate documentation.

Qualifications

• Must be a current RN licensed in Pennsylvania.
• 1+ years of recent Minimum Data Set (MDS) assessments and reporting.
• Strong understanding of Medicare/Medicaid regulations, including RAI.
• Excellent communication, interpersonal, and organizational skills.

Responsibilities

• Conduct comprehensive assessments of residents to identify medical, social, and psychological needs.
• Develop and implement individualized care plans addressing residents' needs and goals.
• Coordinate with physicians, therapists, and social workers to ensure comprehensive care.
• Ensure smooth transitions between hospital, rehabilitation, and long-term care.
• Monitor progress and adjust care plans as necessary.
• Maintain accurate records of assessments, care plans, and resident outcomes.
• Participate in quality improvement initiatives to ensure regulatory compliance.
