Job summary

Brevillier Village Nonprofit Senior Housing and Health Care in Erie, PA seeks a Registered Nurse Assessment Coordinator (RNAC) to lead resident assessments and care planning within the Nursing department.

You will coordinate MDS processes, complete Medicare/Medicaid paperwork, audit nursing documentation, train staff on documentation standards, and ensure compliance with evolving regulations. This is a full-time leadership role with varied hours and a focus on impactful resident care.

Qualifications

• Current or pending Pennsylvania RN license with 2-3 years’ experience in long-term care.
• Proficiency with computers and willingness to learn insurance programs.
• Strong clinical nursing and assessment skills.
• Self-motivated and up-to-date with changing regulations.
• Good employment/attendance record.
• Ability to stand, walk, sit, squat, bend, push/pull, lift 25 lbs alone or 50 lbs with assistance.
• Experience conducting chart audits and training staff on documentation.
• Coordinate MDS and care planning processes to ensure timely assessments and current care plans.
• Prepare paperwork for Medicare, Medicaid and other regulatory agencies.
• Successful completion of training period.
