Wesley Enhanced Living in Philadelphia is seeking an RN Assessment Coordinator (RNAC) to perform federally mandated MDS assessments and develop individualized nursing care plans for residents in a long-term care setting. You will educate the interdisciplinary team and ensure documentation supports appropriate care and reimbursement.

The role emphasizes CMS RAI procedures, collaboration with social workers, and ongoing record maintenance.

Qualifications

• Knowledge of MDS and CMS assessment processes.
• Current RN license and CPR certification.
• Ability to educate and coordinate interdisciplinary teams.

Responsibilities

• Ensure accuracy of MDS sections and clinical documentation to maximize reimbursement.
• Communicate resident assessment details to insurance companies to optimize care days.
• Educate the interdisciplinary team on assessment results and care plans.
• Lead joint care conferences with social workers.
• Follow CMS RAI procedures for admissions assessments.
• Develop comprehensive nursing care plans with the team.
• Oversee clinical and financial record keeping and appeals processes.
