Job summary

NewCourtland Germantown Home in Philadelphia is seeking an experienced RNAC to apply clinical knowledge to connect assessments, documentation, and resident care. You will coordinate MDS assessments, review admission changes, and lead care planning with the interdisciplinary team.

You will develop, review, and revise personalized care plans, monitor resident outcomes, and ensure state and federal regulatory compliance.

Qualifications

• Active RN license is required.
• Experience with MDS, resident assessment, and long-term care preferred.
• Knowledge of state and federal long-term care requirements.

Responsibilities

• Coordinate accurate MDS assessments and submissions.
• Assess admissions and changes in resident condition and level of care.
• Coordinate interdisciplinary care-planning process.
• Lead and participate in resident care conferences.
• Develop, review, and revise individualized care plans.
• Monitor resident outcomes and adjust plans as needs change.
• Maintain required documentation and support regulatory compliance.
• Support quality improvement, surveys, and other clinical needs.
