Job summary

Wyomissing Health and Rehabilitation Center in Reading, PA seeks a Registered Nurse Assessment Coordinator (RNAC) to conduct comprehensive patient assessments and coordinate care with an interdisciplinary team. Qualifications

• Current Pennsylvania RN license.
• Experience as RN with focus on geriatric care and assessment.
• Strong assessment and communication skills; ability to work with patients, families, and healthcare teams.
• Familiarity with MDS software, RAI processes, and ICD-10 coding.
• Ability to work in a fast-paced environment and prioritize tasks.
• Strong organizational and time management skills, with attention to detail and accuracy.

Responsibilities

• Complete and submit accurate MDS assessments on time.
• Build and update individualized care plans for every resident.
• Collaborate with doctors, physical therapists, and social workers during care meetings.
• Use correct diagnostic coding (like ICD-10) to secure proper Medicare and Medicaid funding.
• Follow federal and state guidelines to maintain high standards of care.
• Maintain accurate and up-to-date records of patient assessments, care plans, and progress notes.
• Identify risks and address concerns to prevent hospitalization or rehospitalization.
