Job summary

Chestnut Hill Lodge Health and Rehabilitation Center in Wyndmoor, PA is seeking an RNAC Registered Nurse Assessment Coordinator to oversee the assessment process for new admissions and provide ongoing updates. You will collaborate with the interdisciplinary team to develop individualized care plans for residents.

The role emphasizes MDS coordination, reimbursement compliance, quality monitoring, and support for audits. A PA RN license and prior RNAC/MDS Coordinator experience are required.

Qualifications

• Prior experience as an RNAC or MDS Coordinator is required.
• Active RN license in Pennsylvania.
• Strong assessment and critical thinking skills.

Responsibilities

• MDS coordination and timely completion of assessments.
