Job summary

Susquehanna Health & Wellness Center is seeking a full-time Registered Nurse Assessment Coordinator (RNAC) to lead our nursing team on site. Experience in long-term care assessment and a strong background in MDS coordination are essential for success.

Qualifications

• Must possess a current, unencumbered RN license in this state.
• BSN from an accredited college preferred.
• Minimum two years of experience as an MDS Coordinator in a skilled nursing facility.
• Knowledge of rehabilitative and restorative nursing practices in long-term care.
• Proficient in PA CMI.
• Medicare billing knowledge and PDPM knowledge.

Responsibilities

• Lead the nursing team in assessment coordination.
• Oversee MDS processes and coding accuracy.
• Collaborate with interdisciplinary team to optimize resident care.
